Overdose:
Patients should be managed by symptomatic and supportive care following Lariam (mefloquine) overdose. There are no specific antidotes. Monitor cardiac function (if possible by ECG) and neuropsychiatric status for at least 24 hours. Provide symptomatic and intensive supportive treatment as required, particularly for cardiovascular disturbances.

Precaution and Warning:
Some warnings and precautions to be aware of prior to taking this medication include the following:
  • Mefloquine may increase the risk for seizures. If you have ever had a seizure (for any reason), you should tell your healthcare provider before taking this medication. Because of the potential risk for seizures, mefloquine should only be used in people with a seizure disorder if there are no other treatment options.
  • Mefloquine may cause neurological problems, such as dizziness, a spinning sensation (vertigo), ringing in the ears (tinnitus), loss of balance, or emotional or mental problems. These side effects can become permanent. Let your healthcare provider know right away if you develop these side effects, as you should be switched to a different drug right away.
  • Mefloquine may cause serious emotional or mental health problems, even in people who do not have a history of such problems. In some people, these problems continued even after mefloquine was stopped. Potential emotional problems from mefloquine use include:
    • Feeling anxious
    • Feeling paranoid, or mistrustful of others
    • Seeing, hearing, or feeling things that are not there (hallucinations)
    • Depression
    • Feeling restless, nervous, or agitated
    • Confusion
    • Thoughts of suicide.
If you experience any of these reactions while taking mefloquine, contact your healthcare provider immediately. These side effects may become permanent. You may need to stop mefloquine and start another antimalarial medication.

Overdose:

Chloroquine is very dangerous in overdose. It is rapidly absorbed from the gut. In 1961, published studies showed three children who took overdoses died within 2.5 hours of taking the drug. While the amount of the overdose was not cited, the therapeutic index for chloroquine is known to be small.
Symptoms of overdose include a headache, drowsiness, visual disturbances, nausea and vomiting, cardiovascular collapse, seizures, and sudden respiratory and cardiac arrest.


Precaution and Warning:

It has been found that certain strains of P. falciparum have become resistant to 4-aminoquinoline compounds (including chloroquine and hydroxychloroquine). Chloroquine resistance is widespread and, at present, is particularly prominent in various parts of the world including sub-Saharan Africa, Southeast Asia, the Indian subcontinent, and over large portions of South America, including the basin.


Hematological Effects/Laboratory Tests

Complete blood cell counts should be made periodically if patients are given prolonged therapy. If any severe blood disorder appears which is not attributable to the disease under treatment, discontinuance of the drug should be considered.

The drug should be administered with caution to patients having a G-6-PD (glucose-6 phosphate dehydrogenase) deficiency.

Auditory EffectsIn patients with preexisting auditory damage, chloroquine should be administered with caution. In case of any defects in hearing, chloroquine should be immediately discontinued, and the patient closely observed (see Adverse Reactions).
